f. Component/Parts in Stock at All Levels (Depot and Others) Including War Reserves. Upon receipt of this
TB, the materiel condition tags of all items in al condition codes listed in paragraph 7. shall be annotated to read
"TB 1-1520-210-20-41 (UH-1-SOF-98-07), Mandatory Inspection of Rod-End Clevis, Not Complied With".
(1) Wholesale Stock. Upon receipt of this TB, all serviceable items (condition codes A, B, C, D and E) listed
in paragraphs 6. and 7. and located in wholesale depot storage shall be placed in condition code J and tagged with a DD
Form 1576/DD Form 1576-1 (Suspended Tag/Label-Materiel). Do not remove original condition tags.
(2) Retail Stock. Upon receipt of this TB, commanders and others maintaining retail stock at installation level
and below shall contact the supported aviation unit to perform the inspection required by paragraph 8. and the correction

Summary of the Problem.
a. Two problems have been identified on the UH-1 Rod End Clevis (P/N 204-011-136-103):
(1) The Rod End Clevis (P/N 204-011-136-103 or P/N 1615UH1-100-1) is a flight safety part which has been
undergoing fatigue testing to ensure that all vendors parts meet the strength requirements of the originally-qualified
design. Evaluation of test data has shown that parts manufactured by GT Machine (CAGE Code 4T854) do not meet the
strength requirements and shall be removed from service.
(2) A quantity of Rod End Clevises have been found without serial numbers to identify the manufacturer for
traceability purposes. These parts represent an unknown risk and shall be immediately removed from service.
c. The Purpose of this TB is to require a one-time inspection of all Rod End Clevises (P/N 204-011-136-103 or
End Items to be Inspected. All UH-1H/V series aircraft.
